Appointments System

We are offering telephone and face to face appointments with our Clinicians.  Patients can book pre bookable appointments with any GP at the Surgery for non urgent clinical needs, however if you feel its more urgent then you can call at 8am of any morning (Monday - Friday) and request an appointment on the day either phone call or face to face at either of our Surgeries.

Telephone lines are open at 08:00 each day for appointments, Bloxham Surgery opens at 08:00 and Hook Norton at 09:00.

You are welcome to choose to see any of the GPs or nurses. When you request an appointment please let the receptionist know who you would prefer to see. 

Each appointment is for 10 minutes and if you require an extra long appointment and wish to discuss a number of problems please let the receptionist know when booking. This will help the doctor to keep to time and avoid keeping patients waiting for an excessive length of time. There will always be occasions, however, when the doctor is running behind, perhaps having been called out on an emergency or a particularly complicated consultation, when this happens the receptionist will let you know and we appreciate your understanding and patience.

Online Appointments

Please note that we offer appointments via Patient access that can be booked direct by the patient- these appointments are by telephone only at this present time.  Please note that the time showed on patient access might not be the exact time the GP will phone.  The GP assess their clinic and they manage their own workload when they start in the morning.

Enhanced hours

We offer enchanced hourse throughout the week depending on clinicans availability.

Telephone Advice

If you would like to speak to a doctor please leave your number and reason for callback with the receptionist. The doctor will return your call as soon as possible; sometimes this may be at the end of surgery. Our practice nurses are also available to give advice over the telephone.

 

Please note all the incoming and outgoing calls are recorded.

Doctors’ and Practice Nurse Surgeries

Home Visits

Patients are requested, where possible, to telephone before 11am if a home visit is required that day.

We would request that, apart from the genuinely housebound, all other patients attend the surgery rather than request a home visit because of the extra time home visiting takes. On average four to five patients can be seen in surgery in the time it takes to do a single house call. In addition, the care that can be offered due to the lack of adequate lighting, examination facilities and equipment means that you may not receive as good a service as the doctor may be able to offer if you came to the surgery.

Please note that the doctor may telephone you rather than visit you if this is medically appropriate. Ultimately it is the doctors right to decide whether or not a visit is appropriate for a particular set of circumstances.

First Contact Physio

First Contact Physio appointments can be booked via our Reception Team.

 

You can see a specialist Physiotherapist.  Booking an initial telephone consultation with First Contact Physio gives you faster access to expert diagnosis along with advice and exercises you need to start tackling the problem. They can also refer you on to hospital services where necessary, order tests and recommend a GP to prescribe pain relief etc.

First Contact Physio will:

  • Assess you and diagnose what’s happening
  • Give expert advice on how best to manage your condition
  • Refer you on to specialist services if necessary

First Contact physiotherapists can deal with any of the following problems:

  • Soft tissue injuries, sprains, strains and sports injuries
  • Strains or sports injuries
  • Arthritis – any joint
  • Possible problems with muscles, ligaments, tendons or bone, eg tennis elbow, carpal tunnel syndrome, ankle sprains
  • Spinal pain including lower back pain, mid-back pain and neck pain
  • Spinal-related pain in arms or legs, including nerve symptoms, eg pins and needles or numbness

Our Reception Team might offer this as an alternative to a GP appointment, or you can request a phone call from a First Contact Physio.

Primary Care Wellbeing Service –What to expect from us

What is the Primary Care Wellbeing Service?

Primary Care Wellbeing Workers take a person-centred approach in the delivery of 1-to-1 sessions for people who are experiencing mental health issues or other social or lifestyle issues that are impacting on their wellbeing. The support provided aims to encourage and enable people to link in with existing support services, use the support available in their local community, and develop tools to increase their ability to manage their own wellbeing.

How many sessions will I have?

The Primary Care Wellbeing Service offers up to 6 sessions per person. After your initial session, provision of further sessions will be discussed and agreed between you and your Wellbeing Worker.

How often will my sessions be?

We aim for sessions to take place every 1 – 4 weeks, dependent on your preference and the availability of your Wellbeing Worker.

How will my sessions be delivered?

Depending on your GP practice, you can access sessions with your Wellbeing Worker via telephone, video call or face-to-face. If you require further information about these different options, please contact your GP practice or Wellbeing Worker.

Is my Wellbeing Worker available in between sessions?

Your Wellbeing Worker will be available to be contacted via email only to discuss booking, changing, or cancelling an appointment or to discuss any signposting options they have sent you. Your Wellbeing Worker is unable to offer support sessions over email and please note that the email inbox is monitored during working hours only and is not a crisis response service so please do not expect immediate responses to emails. If you need to speak to a Wellbeing Worker, you will need an appointment with them to access this support.

What is my Wellbeing Worker unable to do?

  • Prescribe medication - we are not qualified doctors or nurses.         
  • Provide Counselling or Therapy - but can signpost to services that can.

Cancel an Appointment

It is important that you inform the reception staff if you are unable to attend your appointment, this will allow that appointment to be offered to another patient. If you fail to notify the Practice that you are unable to attend, you will be sent a letter informing you that you have defaulted from your appointment. Persistent defaulters maybe removed from the list.

You can also complete our appointment cancellation notification request form. This can only be used if your appointment has been arranged for more than 24 hours in advance. (excluding weekends and public holidays). You can also text to cancel your appointment.

Text Reminder Service

We have a texting service which allows you to receive confirmation and reminders about your appointments.

To have this service you will need to register by completing a consent form.

Please remember to update your contact details with us when you change address, telephone numbers and email address.

Chaperone Policy

The practice has a chaperone policy which allows for patients to be appropriately chaperoned during certain consultations, please ask a receptionist if you would like to request a chaperone when booking your appointment or the clinician at the time of your consultation.

First Aid Unit Clinic (FAU) - Enhanced hours. Only at Chipping Norton Health Centre

We are able to offer appointments at the new First Aid Unit (FAU) for all patients (adults and children over 1 years old) with enhanced access hours at Chipping Norton Health Centre.


FAU appointments will be available on weekday evenings from 18.40–20.00 and on Saturdays from 09.00–17.00. They will be 20-minute appointment slots and ALL be face-to-face.

Please note: This is NOT a walk-in service and patients will not be seen without an appointment. 

For data protection purposes, it is really important our patients are aware they are being seen at a clinic which has been contracted to provide this service on behalf of Bloxham & Hook Norton Surgeries and that patient's medical information will be shared with the 3rd party. 

First Aid Unit Clinics (FAU) patient data will be shared with South Central Ambulance Service (SCAS). When Bloxham & Hook Norton Surgery book an appointment, it is for attendance at the FAU in Chipping Norton. You should also be aware that the service is being provided by South Central Ambulance Service (SCAS) on our behalf.
